1. Download the following four documents
   • Medicinal Information Form
   • Parental Permission and Medical Release Form
   • Photo Release Form
   • Swim Classification Record
2. Fill out all four documents on your personal device
3. Save all documents on your personal device
4. Click on the Begin Registration button below. Completed documents can be attached to your online registration by clicking the ‘Add File’ link on appropriate questions.
